Lymphoedema

Lymphoedema occurs when the lymphatic system does not function properly. It can result in long-term swelling of one or more parts of the body which can be extremely debilitating if not addressed early. Physiotherapy can manage symptoms of lymphoedema provided a diagnosis is made and intervention commences in the first (reversible) stage of the condition. However, reduction of the oedema can occur at a later stage, which will greatly improve the quality of life.

Role of physiotherapy

In treating lymphoedema, physiotherapists aim to restore the equilibrium between the load and the carrying capacity of the patient's lymphatic system in order to reduce oedema, restore limb function, and prevent further complications.

To do this, specific therapeutic techniques are used, including Complex Decongestive Physiotherapy (CDP) or Complex Physical Therapy (CPT).

All CDP/CPT treatment includes education on care of the affected limb, including skin care and hygienic measures designed to improve the skin condition and prevent infections.

In addition, depending on severity, CDP/CPT may include:
bullet Manual lymph drainage - a massage technique designed to improve the uptake of excess fluid, increase the carrying capacity of the lymph vessels and reduce oedema.
bullet Compression therapy - designed to increase tissue pressure in order to maintain the reduced level of oedema achieved by manual lymph drainage. This may be in the form of a compression garment or an intensive period of bandaging and manual lymphatic drainage followed by a compression garment.
bullet Remedial exercises prescribed and reviewed according to the patient's needs and designed to improve the function of the lymph system.
An early diagnosis may find the patient requires only education on care of the limb and an exercise program to reduce and control the oedema. Prophylactic education about lymphoedema to those with a limb at risk (i.e. post removal of, or radiotherapy to, lymph nodes) may reduce the incidence of lymphoedema.

Benefits of physiotherapy

Physiotherapists can assist medical practitioners achieve good clinical results in a cost effective way. Essential elements include:
bullet an early accurate diagnosis of the primary condition;
bullet an early accurate diagnosis of associated conditions (e.g. cardiac failure, hormonal imbalance, cancer, deep vein thrombosis, and spinal lesions);
bullet a detailed knowledge of anatomy, physiology, and the pathology of lymphoedema and associated conditions;
bullet expertise in diagnosis and assessment, manual handling, massage, bandaging, and exercise prescription;
bullet appropriate treatment by a highly skilled practitioner with a good understanding of the benefits and contraindications of specific treatment methods.
